Model Intake Form

Thank you so much for wanting to model for BDL Photography. I'm dedicated to providing the most comfortable session as possible while also capturing your beauty as an indivual.

I'm looking for indivuals who demonstrate the following:

• Must be 18 years of age or older
• No previous experience necessary
• A fun and light-hearted attitude
• Ability to listen and follow verbal directions as well as mimic poses
• Someone who wants to have fun!
